For a person to act as a Godparent for Baptism, he or she must be at least sixteen years old, must have already received all the Sacraments of Initiation (Baptism, Holy Eucharist, Confirmation), and must be living in a way that demonstrates that one's faith is strong enough to be able to fulfill the responsibilities involved with being a Godparent. A Godparent who is married must be married in accord with the laws of the Catholic Church. ~ cf. Code of Canon Law, can. 874
For Holy Angels parishioners, the pastor is normally responsible for determining that the parishioner understands the Godparent’s role and is qualified to assume it. If qualified, the pastor, or his designate, is to provide a certificate of eligibility for the designated Godparent.
The form below is to be completed by a Holy Angels parishioner seeking to serve as a Godparent at Baptism in the Catholic Church; whether the celebration of Baptism is taking place at Holy Angels Parish or any other Catholic parish.
